We assist our clients with the daily activities of life from the moment they wake up through to bedtime and beyond. Assistance is personalized, based on a pre-agreed needs assessment. The assessment will be updated annually or when changing needs become self-evident.
The scope of potential assistance includes, but is not limited to assistance with:
· Getting in and out of bed
· Dressing and undressing
· Bathing and personal hygiene
· Medication Administration
· Mobility

We aim to create a safe, comfortable, and serene environment to allow our clients to remain in their homes and avoid institutional care for as long as possible.
At Home Care with a Heart, we believe that an elderly person’s mental well-being is as important as their physical well-being. They should be happy and contented in their golden years, not lonely. This is difficult as they lose in-person contact with friends and relatives. They need somebody to talk with when they want to talk. A major part of our carer's job is to provide your loved ones with companionship and conversation so that they remain emotionally engaged. We also focus on mental stimulation with board games, puzzles, and reading.
Though we are not medical professionals, we monitor and log your loved one's health daily. We measure basic vitals, including blood pressure, temperature, and weight. We look for evidence of illnesses or changing behavior and report this to our family members. We respond to alarm systems to control wandering and fall risks. All health data is kept confidential but will be shared with immediate family with permission.
We ensure daily medications are taken on time and logged.
We have contract Private Nurses on retainer. They can visit and asses our client’s health or provide ongoing, in-home skilled nursing care.
We help your loved ones remain physically active through walks, light exercise, and stretching. Some of our carers will exercise alongside their clients.

We can make your loved ones' lives a little easier by shopping for groceries and preparing healthy meals and through light housekeeping including:
· Tidying up to avoid tripping hazards
· Cleaning kitchen counters and cooking areas
· Dusting surfaces
· Vacuuming once or twice a week.
· Light bathroom cleaning.
· Changing bedding
· Taking the trash out.
· Watering plants
